If you share this vision, we invite you to apply for the opportunity of Transition Care Coordinator. The ideal candidate for this fast-paced, diverse position will have a proven clinical and customer service background, an outgoing personality with a true heart for people and a strong desire to succeed. The successful candidate will become the lead representative of MHRC by building professional relationships within the community.
KEY RESPONSIBILTIES:
- Represents MHRC as the direct liaison to potential residents and their families; to physicians and other healthcare professionals; to community organizations; and to the general public
- Clinically evaluates and processes potential residents from hospital or in-home referral to placement within MHRC
- Works directly with hospital discharge planners to assess potential residents' clinical needs including medication, special equipment and/or placement on specialized units
- Coordinates logistical arrangements, education of the patient and family, and ensures communication among the health professionals involved in the transition while communicating closely with MHRC staff to ensure a seamless transition to our community
- Works directly with patient and/or family to coordinate necessary admission paper work while providing a communicable understanding of Medicare, Medicaid, insurance and issues important to senior citizens.
- Works with the director of marketing to establish and achieve MHRC marketing, census and sales initiatives while maintaining accurate records
- Perform duties consistent with all State and Federal regulatory requirements.
QUALIFICATIONS
- 3 to 5 years of recent clinical experience preferably as LPN or RN.
- Strong leadership and communication skills and the ability to make decisions with minimal supervision
- Marketing or sales background preferred
- Strong computer competence, particularly with Microsoft Office suite software applications
- Ability to work flexible schedule
